Eligibility: Administrators and Faculty with an annual half-time contract or more are eligible for enrollment within the first 30 days of commencing work at California Lutheran University.

Option 1:

BLUE SHIELD - PPO
(Rates effective June 1, 2013.)
Description: Available to all regular, full-time and half-time faculty and administration.
Employee must enroll through the Human Resources Office.
Cost: $31.18 per month - Employee only.
$453.95 per month - employee + spouse only
$822.41 per month - employee + family
$315.99 per month - employee + child(ren) only

Option 2:

BLUE SHIELD - HMO
(Rates effective June 1, 2013.)
Description: Available to all regular, full-time and half-time faculty and administration.
Employee must enroll through the Human Resources Office.
Cost: No cost for employee coverage.
$265.66 per month - employee + spouse only
$618.35 per month - employee + family
$150.03 per month - employee + child(ren) only

Option 3:

KAISER - HMO
(Rates effective June 1, 2013.)
Description: Available to all regular, full-time and half-time faculty and administration.
Employee must enroll through the Human Resources Office.
Cost: No cost for employee coverage.
$283.24 per month - employee + spouse only
$687.48 per month - employee + employee + family
$148.49 per month - employee + child(ren) only
